Aged Care Coordinator (Case Manager)- Brisbane Southside

Fantastic opportunity for a HCP Manager/Coordinator to join this well-reputed, growing Community provider in Brisbane Region!

Opportunity for an experienced Case Manager with home care / Case Management experience to join a growing Community / Home Care organisation. You will be responsible for providing support and advice to the clients and their families to ensure timely admission into the service. You will oversee all telephone enquiries, arrangement of home assessments, ensure compliance,general operational duties & management of staff. You will be working alongs ide a team of pro-active coordinators and report to an experienced Manager.

This is the perfect opportunity for a professional with experience within home care to be a part of a growing organisation. Within this group there are excellent prospects for career advancement.

Requirements:

· Minimum 2 years working experienceof In-home Aged Care or related qualifications.

· Able to speak English, Mandarin and Cantonese.

Solid understanding of client centred, community-based care service delivery.
Previous experience in a HCP role is necessary
Demonstrated effective interpersonal and written communication skills
Experience within Aged Care/NDIS industry required
Knowledge of continuous quality improvement activities
Effective interpersonal and written communication skills
High personal and professional ethical standards
Understanding of Home Care legislation
